Notice of Privacy Practices

We are required by law to keep your protected health information private, to give you this notice of our legal duties and privacy practices, to follow the notice currently in effect, and to notify you if a breach affects your unsecured health information.

• Treatment: providers use your questionnaire and history to decide on treatment, and we send prescriptions to our partner pharmacy.
• Payment: we process payment for your orders. Our payment processor receives billing details and amounts only, not what you ordered or why.
• Health care operations: to run our service, review quality and train staff.
• Business associates: companies that help us operate, such as hosting providers, under written agreements to protect your information.
• As required by law, including to public health authorities, for health oversight, for law enforcement or court orders when legally required, or to prevent a serious threat to health or safety.

We will not sell your health information or use it for marketing without your written authorization. Any other use requires your written authorization, which you may revoke at any time.

• See or get a copy of your health and prescription records.
• Ask us to correct information you believe is wrong or incomplete.
• Ask us to limit what we use or share, or to contact you in a specific way. We will consider every request and may say no where the law allows.
• Get a list of certain disclosures we have made of your information.
• Get a paper copy of this notice at any time.
• File a complaint with us, or with the U.S. Department of Health and Human Services Office for Civil Rights, if you believe your rights were violated. We will not retaliate against you for filing a complaint.
